To register for classes please call    our office at 513-729-2550 or use the form the the and email  to driveredacademy@msn.com 
 with the following information: 
 
Student's Name and:
  • Full Address
  • Phone Number
  • High School
  • Date you plan on starting
  • Location attending

 

Once you are registered with our office you can print out the following forms and bring them to your first class along with: 
  • Copy  of your temporary permit 
  • Pencil and folder
  • Payment in full or deposit
  • Copy of you Report Card
  • Copy of forms below
